INFORMED CONSENT
Prior to each massage session, the treatment plan will be discussed with you. When you book your first initial treatment you will be sent a confirmation email one week prior and a second reminder confirmation email the day before your appointment.
Appointments must be booked a minimum of 24 hours in advance
Last minute/ same day appointments are NOT accepted
24 hours prior to your appointment, you will be asked to fill out a Health History Form as well as a signed consent to treatment. Both of these forms are MANDATORY! If either of these forms are not completed treatment will NOT be provided. Clients who I have not seen for at least one year will also be asked to update and fill out their health history forms.
When you book an appointment, please be considerate and note that a set amount of time has been allocated for you, not just for the hour of treatment provided but also time it takes for travel. Missed appointments prevent from accommodating other clients. Please understand that Therapists only get paid when they deliver a service.
*24 HOUR NOTICE IS REQUIRED FOR ALL CANCELLATIONS *
Payment of the FULL treatment fee/ service booked will be charged to the credit card provided on file for late cancellations or "no-shows" unless:
- appointment can be filled by another client
- COVID-19 cancellations due to symptoms or exposure are cancelled without question
-I recognize that there are circumstances that are out of your control (sudden illness, family emergencies, testing positive for COVID-19.
- Missed appointment fees are NOT eligible for direct billing or insurance compensation.
